About this toolWhat is AI Prompts for Nurses?
Nursing work produces a steady stream of structured communication: the SBAR handoff at shift change, the call to a provider when a patient deteriorates, teaching a patient to use an inhaler, updating a worried family, writing an incident report that sticks to facts. This kit holds ten prompt templates built around those moments, each with fields for the details that matter and a finished prompt you can paste into any AI assistant.
The prompts keep the model in a supporting role. It organises what you observed into SBAR or care-plan structure, writes teaching material that ends with teach-back questions, and lists what is missing instead of guessing vital signs or assessments. Before you copy, a privacy check looks for patient names, room-and-bed style details with dates, MRNs and phone numbers and masks them. Everything happens on this page in your browser.
- Ten nursing templates from SBAR and care plans to incident wording and student practice
- Teach-back questions built into every patient teaching prompt
- Identifier scan that masks titled names, MRNs, dates, phones and emails
- What-not-to-paste list and a nursing review checklist next to each prompt
- Starred favourites kept in your browser and a Markdown copy of the whole kit

How to use AI Prompts for Nurses
- Enter your unit or setting, such as medical-surgical ward or community nursing, and pick your region.
- Choose a template: SBAR handoff, escalation call script, teach-back teaching plan, discharge education, family update or one of the others.
- Type your observations and notes, keeping out names and record numbers, or load the fictional example to see how it reads.
- Mask anything the privacy check flags, copy the prompt and paste it into the assistant your employer allows.
- Check the draft against your own assessment, the chart and local policy before you say or file anything.
Worked examples
Escalation call to a provider
Input
Post-op day 1, new tachycardia, falling urine output, observations and your concern
Output
A 60-second SBAR script with a clear request and a read-back line
The script ends with a specific ask (review within 30 minutes, orders for fluids or bloods) because vague escalation calls are the ones that stall.
Teach-back for a new inhaler
Input
Device type, dose schedule as prescribed and the patient's health literacy concerns
Output
Step-by-step teaching script plus three teach-back questions
Teach-back asks the patient to explain in their own words. The questions it drafts let you confirm understanding rather than asking "any questions?".
Background and insight
SBAR (Situation, Background, Assessment, Recommendation) originated in military communication and was adapted for healthcare in the early 2000s as a way to make urgent conversations predictable. A model is good at reshuffling rough notes into those four boxes; it is not able to judge whether a patient is deteriorating. The escalation template therefore asks you for the observations and your concern, and it drafts the words you say, with the recommendation in your own terms.
Care plans are where AI drafts need the most care. The care-plan template asks for nursing problems, goals and interventions as a draft for your review and tells the model not to add interventions outside what you list or your unit's protocols. Nursing diagnoses taken from a formal taxonomy should be checked against the version your organisation uses, because models often paraphrase labels.
Patient data rules are the same as for any clinician. In the US a vendor that handles protected health information needs a HIPAA business associate agreement, and in the UK and EU health information is special category personal data under GDPR Article 9. Pseudonymised text is still personal data under GDPR Recital 26, so masking reduces risk but does not make a consumer chatbot an approved place for patient records.
Common use cases
Shift handoff
Turn scribbled notes into a consistent SBAR handoff so nothing important is lost at the 7 o'clock change.
Escalating concerns
Rehearse what to say to a provider or rapid response team when a patient is getting worse, with a clear recommendation.
Patient and family teaching
Write medication sheets, discharge teaching and family updates in plain words with teach-back checks.
Nursing education
Students and educators generate practice questions with rationales and summarise a long policy into a checklist.
Tips and limitations
- Vital signs, early warning scores and drug doses must come from the chart, not from the draft. Re-read every number.
- Incident reports should state facts in neutral language. Ask the model to remove blame and opinion, then check it did not remove facts too.
- Medication education sheets must match the product information and the prescription; confirm against your drug reference.
- For students, AI-generated practice questions can contain wrong answers. Check each rationale against your textbook.
- Follow your employer's AI policy and your regulator's guidance on documentation and confidentiality.
Frequently asked questions
Can AI write my nursing care plan?
It can draft one from the problems, goals and interventions you give it, which saves typing. You still assess the patient, choose the interventions and check the plan against your unit's protocols and the nursing diagnosis taxonomy your organisation uses.
Is it safe to put SBAR handoff notes into a chatbot?
Only with identifiers removed, or in a tool your employer has approved for patient information. Use the privacy check to mask names, MRNs and dates, and leave out room numbers or details that point to one person.
Will the escalation script tell me whether to call a rapid response?
No. Whether to escalate is your clinical judgement under your hospital's criteria. The template only drafts how to say it clearly once you have decided.
How should I word an incident report with AI help?
Give the facts in order and ask for neutral, objective wording. The template tells the model to remove opinion and blame without changing what happened, and to list anything unclear for you to confirm.
Can nursing students use the practice question template for NCLEX revision?
Yes, as practice. Check every answer and rationale against your course materials, because models sometimes mark the wrong option as correct.
Does the page keep a copy of what I type about patients?
No. The text stays in the page while it is open and is not sent or saved. Only the list of starred templates is stored in your browser.
Is there an in-depth guide to AI Prompts for Nurses?
Yes. “AI for Doctors and Nurses: Safe Prompts for Notes, Letters and Teaching” covers the background, how it works, common pitfalls and worked examples. Read the full guide.
